Family members or policyholders can download the pre-approval form on behalf of the Insured Person and submit it to the doctor for completion, and then email it to claim@bowtie.com.hk for application.

If the Insured Person is underage or unable to sign the pre-approval form due to an accident, a family member or guardian may sign and apply on his/her behalf.
