In what situations would pre-approval not be granted?

Bowtie reserves the right to reject your application if the following situations are discovered, including but not limited to:

  1. The treatment or surgery is not medically necessary
  2. The charge of the doctor exceed reasonable and customary levels
  3. The reason for application falls under policy exclusions
  4. The pre-approval information filled out by the doctor is incomplete
  5. Insufficient admission information - emergency admission
