1. Log onto Bowtie’s customer portal
2. Download relevant critical illness claim form
3. Invite your doctor to complete the form

(if the form for the diagnosed critical illness is not available for download on the platform, please contact our Customer Service Centre).

Please submit your claim to us by uploading the form and other supporting documents (such as discharge summary, referral letter, diagnosis and laboratory reports issued by a registered doctor or specialist) through the platform within 90 days of the diagnosis.

Please note that for policyholder who buy before child is born, personal information and birth certification of the insured newborn baby are required to proceed claims. We will handle the claim once confirmed the information.
