If you would like to get a preliminary assessment of the claim amount, we suggest that you have the doctor fill out the ‘Medical Expense Estimate Form’ during the medical examination or before the surgery. We will then estimate the amount you can be reimbursed based on your current coverage, and provide the estimate within one week for your reference.
